The Art of Fallout 4 States the following of this car: "The large sedan seen here returns from Fallout 3 [...]" Also: "Sedan: A reimagining of the classic two-door Corvega from Fallout 3." Though the car takes design cues from previous Fallout vehicles, most notably the front and sides, the real life inspiration for the rear end is definitely the 1951 Buick XP-300, as suggested by kalu.

A yellow example can also be seen at the Red Rocket fueling station as the protag first meets Dogmeat. As such it's definitely in the game as an actual model. In this trailer it appears to have a front fascia similar to the Corvega, and they might be closely related.

The grille shape is vaugely similar to certain Chrysler models from 1967–1960, like the Chrysler 300. The rest of the car is difficult to pin down, as it is a 21st century evolution of 50's aesthetics. The fins remind me of the 1958 Oldsmobiles however.
